*Full Membership/Family receives 2 seats with membership as well as free seats for children under the age of eighteen, and Single Adult membership receives 1 seat with membership as well as free seats for children under the age of eighteen. Kindly fill out the necessary information below so we know how many additional seats you would like and what services those guests will attend.  

Be sure to list the names of all attendees in the section below your selection, so we can prepare an accurate list for security check-in. This is mandatory.

If you would like to enhance your High Holy Day experience, the Machzor Lev Shalem is on sale in the office for $45.  They will be available for pick-up at the office.
